GI Care Coordinator - HFMG, Full-Time
To be fully engaged in providing No Harm / Quality, Customer Experience, and Stewardship by assisting in medical care of patients and supporting front desk functions as needed under the direction of the physician and clinical associate/practice manager. Works collaboratively with multi-disciplinary care team across the continuum of care to help organize care and disease management to high risk patients identified in the ambulatory setting. Focuses efforts on patient outreach and care assistance for a panel of patients to achieve optimal outcomes and promote wellness, decreasing preventable emergency department visits while improving patient satisfaction.

* Administers all medications as ordered and assures all documentation is noted. Maintains current CPR certification ? Knowledge regarding location of emergency equipment/supplies and how to use such equipment/supplies.
* Ability to perform basic computer skills pertinent to job performance.
* Helps to identify which patients in the specialty care practice have ongoing care coordination needs for their specific condition under direct physician supervision.
* Assists with coordination of GI therapy, medications and care locally or to outside physicians/tertiary centers (Florida Hospital, Shands, Mayo, Jackson Memorial) under direct physician supervision.
* Educates and monitors patients on treatment protocols for Hep C, Hep B, Crohn's Disease, Ulcerative colitis and bacteria. Educates and connects to other care providers and other resources to enhance care.
* Manages patient compliance and follow-up of therapy through protocols, office visits and required labs for authorized treatments as per disease guidelines.
* Communicates effectively with customers (patients, physicians, and other Health First Associates) practicing excellent customer relation skills at all times while organizing and directing daily medical assistant functions.
* Mentors co-workers and students' technical development.
* Schedules procedures for patients with chronic GI conditions.
* Performs other MA duties as needed to support clinic staffing needs.
* Informs the patient and family regarding coordination of their care per physician directive and documents this information in the electronic medical record. Serves as GI care patient contact for team related to condition and facilitates access to services.
* Serves as the liaison between patients, families, physicians and clinical staff by advocating for patient and families then responding to and facilitating resolution of patient/family questions and concerns.
* Utilizes supplies/equipment in a proficient and cost effective manner when performing various procedures.
* Completes all required documentation and/or required forms
* Demonstrates effective time management skills
* Remains flexible and adaptable to changes in workflow, processes, etc.
* Accepts changes to the schedule or workflow based on patient volumes.
* Collaborates with insurance companies and HFMG billing office by assisting with appeal for services rendered or to be rendered to ensure payment. Advocates for patients denied by insurance who have a legitimate need for GI therapy or medication.



* High school graduate or equivalent
* Must hold current AHA, BLS Healthcare Provider Completion Card prior to start date and maintain.
* Graduate of an accredited medical assistant program, or 5 years' experience with certification from one of the agencies noted below.
* Current certification with:**AAMA (American Association of Medical Assistants)**NHA (National Healthcareer Association)**AMT (American Medical Technologists)**NCCT (National Center for Competency Testing)
* 2 years of experience in managing GI therapy preferred.
